Cancellation Policy & Waivers:
At time of appointment, you must sign and return it before appointment it will be sent via email or postal mail in the forms of a digital copy by email word doc/pdf , or a hardcopy, you can also send an email back with your name and info to confirm if you can't access these functions, you can use a verbal agreement by phone at time of appointment. This must be done to confirm appointment, start a session through our wavier process. Stating you accept these terms. There are no extra fees charged for cancellations, we would have to refund money if already paid for service or reschedule your appointment at another time (your choice).Please cancel no later then 7-14 days before appointment date, unless it's an major emergency situation occurs from either party. All participants must be at least 18 years of age, only acceptation is that children under age of 18,such as teens 13-17,must have waivers signed by both parent or guardians & the child, all parties must attend the session together, this cannot be completed without the presence of another adult at No extra charges, to attend with children.
